I have an LSI Logic (used to be AMI, oem'd to Gateway) 438 RAID controller. I attempted to flash this board with their mflash utility which auto-detects the card. The flash 'succeeded' but no card post on restart. I assume that because it was gateway oem it didn't like the file. Neither company is any help. I have a correct (albeit older) image from gateway now, but since their mflash utility has to auto-detect the card (and no longer does) I can't reflash it.

Are there any utilities out there that can help me force the correct flash to this card's bios? Thanks!!

Try UniFlash with -PCIOR parameter. But I know only one card that works with this - Adaptec 19160 SCSI.

How can I confirm what bus/device/function my card is installed on? Thanks!

According to the Manufacturer and Device ID numbers - if you have Award BIOS, you can see them in the table before boot.
