I've also heard that Anti-Virus software can cause this in Windows, but usually when you reboot, the clock resets. The BIOS clock isn't changing, it's just that Windows "time" gets behind.

On ATX boards, if the clock gets a couple of hours behind when the PC is powered off, it can be caused by bad PSU. Clock runs from +5V StandBy power from ATX power supply.
