Motherboard Identification

Hi!. I have a problem on my old AT mobo. Its dead and I cannot identify the motherboard model and its manufacturer. Here are the only hints i have: (pls help!)

1.) At the back of motherboard, i can only see El pina BA E-VO 5 IL086.
2.) At the front of motherboard, i can only see rev.1.2
3.) The BIOS is AMIBIOS
4.) Chipset is ALI M1621 and ALI M1543.
5.) Slot 1 type.
6.) AT form factor.
7.) AT/ATX support.
8.) 1 - AGP slot / 3 - PCI slots / 2 - ISA slots
9.) On-board USB
10.) On-board Sound Pro (HT1869V+) soundcard.

Pls do help me.

Maybe M725 or M729, use the ... do not know ... section
