PC Chips Motherboard?

How-to identify your motherboard ?

My only hint for this old motherboard are the following:

1.) PCB V1.2A
2.) MB-686LX1 sticker
3.) Intel chipset
4.) Slot 1 type
5.) AMI bios sticker
6.) 1-AGP slot, 3-PCI slot, and 2-ISA slot
7.) 2-SDRAM slots and 2-SIMM slots
8.) AT/ATX type of PSU
9.) ITE Super I/O chip
10.) SoundPro HT1869V+

I need help if this is an old PC Chips. I need also its bios file and manual.

Are the ones we know for sure. I still have to complete the list...
